I’ve not looked at the raw data, but never would have guessed the following statement to be a reality.
 

“While intentional acts traditionally aren’t included in air-crash statistics, they would be the second-largest category of deaths worldwide if they were, according to data compiled by Bloomberg.”

It’s because of how safe Commercial flying has become, the accidental death rate is so low

I wonder though if that includes all intentional acts, there have been more than one that the host countries have refused to accept it was an intentional act.

It's not horribly surprising.  If you look at young adults (not a perfect comparison, of course), murder, suicide and accidents are the biggest causes death (murders and suicides combined are outnumbered by accidents by 2:1 or so).  Given that few people are dying of heart attacks or cancer while on airplanes, it's hard not to imagine there is risk there once you reduce the number of accidents.
