Got a call today that the plane will be ready for pick up tomorrow.  The vacuum pump disintegrated and the #2 cylinder base o-ring blew out.  Along with the fill tube being cracked at the base.  The rigging was fixed I had three joints that were slopped out, and replaced some more hardware while  in the belly and the wings.  I am guessing this is just from wear and tear and age.  My concern is why the o-ring or does this just happen?  I know the vacuum pumps are an issue when they get old and apparently it hasn't been looked at just like the magnetos.

He stated it was the base o-ring, not the oil control ring. A bad base o-ring would cause leaks like the OP was having.

Is your breather tube clogged?   Has the cylinder been off and back on?   Might have been nicked when being put on.   Proper torque of the cylinders.....

Do base O-rings just go?  I would think if I had an oil control ring issue I'd be smoking like a freight train.  If I had cylinder issues I would think I'd be pushing oil from the vent.  So why did the base o-ring fail.  Does it sit in a groove or is it a squeeze fit?  Sorry for all the questions but I O-rings in my business don't just fail? Usually the fitting comes loose or an o-ring is reused then it fails.

No I cleaned it myself weeks ago and even serviced my airwolf catch system.

They shouldn't just fail. Most likely damaged at installation as others have said. They should be lubed with oil when they are put on. They sit in a very small groove in the crankcase.
