The Dreaded 8500-8 form (Medxpress)

My medical doesn't come till next May - but I started thinking about the process. 

 

Obviously it's best to be consistent on the information that is put on the forms, but I neglected to memorialize what I put the last time.  The great folks at CAMI have a (to me at least) little known service for those of us that are less structured than we would like to be.  With a written faxed request they will send (free of charge) a copy of the last submittal.  Mine was a two week turn.  The response comes certified.

 

Now I don't dread the form any more!

Just got my Med done last week used the FAA site for the second time and all my info came up from the first time except the medical history stuff needed to be updated just to keep it current. got my confirmation number and went to the doc's for the medical

So how does one sign in again? It's been two years, I don't remember the website, my id, nothing.

Yep the user information came up for me as well - but none of the history - that's what I want to be consistent on.

It has apparently decided that I'm a 680-hour student pilot. Hopefully my new AME can straighten things out.

Filing the form via the Internet is compulsory now. The AME will not take you without it. I think it is a form of testing your intellect. I flunk it twice before I got it right.

Tip of the day: After completing the form and inputing your password to get your conformation #........if you get an error message saying your password does not match the logged in user: If your password is more than 12 digits long, ONLY use 12 and leave the rest off.

I just wasted an hour trying different computers etc. :-( At least the internet help desk was quick and knew the exact issue.

I discovered a month prior to my April medical that the FAA site does not reload the doctor visit history.

 

To add insult to injury, they want three years even though the medical is good for two. Go figure.

I laboriously reconstructed my medical history, but this time I put it in Excel.

 

Thank god for the Internet (physician names/addresses/etc.).

 

The next time I even think about visiting a physician I will remember not to. Thank you FAA for preserving my health.

Our printer doesn't matter. The doc does his thing, types in his stuff then prints it out.

I print my form off each year. There is also an option to save as PDF, I do that as well. Lastly every few years I request a copy of my full FAA Medical records (which they must provide you free)

It has a copy of EVERYTHING the FAA has on me medically. I fly on special issue. This year I'm requesting after more than 15 years of stability to be released from the special issue. I had cornea transplants 15 years ago! I'm corrected 20/20..... Well see FAA exam is today! I have multiple letters from my eye Dr and Eye Surgeon who state I'm stable and have been for more than a decade. But one must remember this is the FAA they don't have to be logical or reasonable do they!

Super easy to get your records fax them a one sentence letter requesting a complete copy of your records, they will send them certified mail now due to HEPA.
