Wow, glad you guys are safe!

Any idea if it was the hex plug coming loose from a heat shrunk gasket?  Sputtering engine was the effect of the hex plug coming loose.

In my case I had that checked before the o/h; was not an issue.  The NTSB arranged a bench test and tear down of my fuel system after the accident and a small piece of a torn o-ring was causing intermittent fuel flow. It was likely the issue before the o/h. Shop that did the work said they “weren’t into that section “.

How can they have called it an overhaul yet not have disassembled the entire servo?  Did the FAA follow up with them?

Link doesn't work?

In any case, there was no sputtering. Running one instant, completely dead the next. When I relayed what happened (sudden death; engine came back when I pulled mixture back), every A&P I spoke with went immediately to the fuel servo. The A&P who looked at it had the cowling off and dug into it and determined it was the fuel servo. I wasn't there, but he'll be the one signing it off as air worthy, so... But I'll forward this information to him to ensure he checks it, thanks!

Edit: Sent that info to the mechanic; response: "First thing I looked for."
