MSFPPP – Training Liability Waiver?

I'm doing a small survey of type-specific training programs and whether they use liability waivers/hold harmless agreements. I know for example that the Bonanza PPP uses one (it's available on their website).

For those here who have used the MSF program let me know whether it uses one and, if so, whether you can send me a copy.

When I took the class last year in KCHA, they had to named as an additional insured. Don't recall having a hold-harmless liability doc.

Just wanted my insurance to have their name on it. One phone call and it was added.

Yep, all MAPA has ever asked of me is to be named on the insurance. It's a quick phone call, a fax/email, and no charge. Insurance likes it when we get type-specific training.
