Having to do approaches in low IFR in the middle of the night with a minimally equipped airplane sounds like the opposite of a good time to me.   
 

There must have been intense external pressure on that group to entice them into this scenario.

I don’t see lots of holes in the cheese.  I only see the incredibly poor decision the pilot made by attempting the flight.  Fatigue, lack of proficiency, get-there-itis, inadequate understanding of the weather conditions, improper equipment, and inadequate equipment for the conditions were the Swiss cheese holes created by the pilot himself.

Ultimately, legal currency does not equal proficiency.

the most amazing thing about this accident is that the plane didn’t go down on the first failed approach.
So much poor ADM including attempting flying an RNAV approach to minimums with a portable VFR GPS and even failing to track an ILS on a subsequent approach showed get-there-itis enabled the pilot to get in way over his capabilities that night.
I’d be surprised if there weren’t some large law suits from the pax’s surviving families.
