NTSB inspected the aircraft 5 weeks after the July 2012 event.

Interesting speculations, but I'm still not hearing anything factual that even confirms that there was an engine failure. Based on first hand description provided it could have very well been fuel starvation or some other accessory failure separate from the engine. Maybe it would be more appropriate to wait for some more facts before implying that improper repairs follow the previous prop strike contributed to this incident?

I owned N1310W, a 64E, from 1989 thru it's demise in 2012. Between 2010 and 2012 I experienced 3 incidents where my engine suddenly lost power and/or ran very rough. During this period plugs and injectors were inspected and cleaned, new plugs were installed, ignition wiring was tested, and mags were O/H. I also sent the entire fuel injection system out for O/H. Throughout the 2 year period we found some issues, such as a spec of dirt in an injector or a lead ball in a lower plug. None of these findings were considered serious enough to explain these few incidents, which were few and far between. Between these incidents, the engine ran fine. Then in 2012 I had my final engine out incident (to date). After the off-field landing the 64E was totaled. Later inspection revealed an internal blockage in the fuel injection system THAT THE SHOP THAT DID THE PRIOR O/H SAID WAS IN AN AREA THAT THEY NORMALLY DO NOT INSPECT. The blockage was apparently very intermittent, since the aircraft ran fine 99% of the time and passes a bench test before the incident. It also almost passed a bench test after the incident.

Not to be pedantic, but Jetdriven's talk about "running out of fuel" is "fuel exhaustion ". Not to be confused with "fuel starvation". My engine out in 2012 was caused by a piece of torn o-ring that blocked flow internal to the fuel injection system, resulting in fuel starvation.

You don't have to run out of fuel to experience fuel starvation.

When I was working on my PPL in a C150, I had the CFI turn off the fuel valve, while I wasn't looking, to simulate an engine out. We were nowhere near the airport. I restarted the engine, flew back to KIAG, and fired the instructor. -

Glad everyone made it down with very minor injuries. Sold by Jerry Pressley earlier this year. Wonder if it will go back east for another rebuild? -
This is a potassium lactate aerosol. May result in damage to electronics, has a relatively short shelf life, will freeze under 20F, and does not meet any ANSI/UL standards. Other than that, it will likely work better than a dry extinguisher but not as well as Halon on an electrical fire.

This, from the FR preamble to the AD, is why I made my comment. I hope I'm wrong! "Request To Allow Operators To Determine if the ELT Is Functional Michael Dunn requested that we allow operators to determine if the ELT is functional. The commenter noted his AK-451 ELT was inadvertently set off and it worked. We disagree with the commenter's request. The service information specified in this AD provides instructions for testing the ELT, and we have determined this test is necessary to address the identified unsafe condition."

Before this AD, operators were permitted to conduct this test. Unfortunately the FAA was petitioned to permit operator sign-off, but declined. The operator is not permitted to sign off the AD.

Last Thanksgiving I flew into SLC. Passed on South Valley since the FBO had closed, and rental cars would have been difficult & expensive. As I remember it, KSLC wan't very expensive.

I once ordered #2 fuel oil and got gasoline. The driver swore it was fuel oil, and the BOL was for fuel oil, but fuel oil is UN1202 and the tanker was placarded UN1203. Stuff happens. -

Did they receive and off load an entire auto gasoline tanker? I ask because that's possible, and is quite different from mixing the two fuels. That can result if a driver hooks to the wrong tanker, and disregards the tank # on his bill of lading. I worked in the petrochemical industry for decades and have seen this happen a few times with other commodities. Quite different from contamination due to mixing. That can happen even with a dedicated 100LL fleet. -

I assume that, as you say, you "know" that this sometimes occurs. I'm surprised by this. I had thought that in North America 100LL has to have it's own dedicated equipment, which is one of the factors in it's high cost. The tetraethyl lead is quite persistent in pipelines, tanks, and hoses/lines, which will cause contamination of other fuels. -
