At what pressure altitude do you start?

And For exemple from sea level to FL160

Do you set +100degF rich to the peak …? (I have Garmin G500 Xi in my M20J)

My home field is 4000’. I lean slightly for takeoff, and about every 2000’ thereafter on the climb up to 16,000’. For climb, I target best power plus about 100*F for added cooling. Once additional cooling is no longer needed, I’ll run best power EGT and 2600 or 2700 RPM for the climb. The IO-360 is a pretty forgiving engine to operate, but you’re right to lean as you climb.

On a sea level takeoff, full rich, note your egt.  Lean as you climb to maintain that egt.  It works well.  That egt is now your target egt for any takeoff at any altitude.

Just pick one cylinder to avoid confusing yourself as they will all be a little different and swap places occasionally as you climb.

Exactly that. My takeoff EGT at 0 MSL is usually 1350F on the hottest jug. I adjust mixture every 1000 ft to stay close that number. 
1000-ft checks also help continuously monitor CHT and adjust climb rate for the most efficient climb.

I need to use the hidden cylinder 3 or 4 as a reference...

I don't really have a science to it. I take off at 1200' MSL. I have it in full rich. After I pull back to 26/2600 I leave it in full rich til about 3K' then I start arbitrarily leaning it. I spin it a couple times, wait another thousand feet and spin it another time. When I get to cruise altitude I just run it ROP for a little bit and then close up cowl flaps, lower to 2400 RPM and then start my Lean of Peak estimating. 

 

I'm sure there's a more precise way of doing it and I think I might try the target EGT method mentioned earlier.

2600 is a lot quieter than 2700 is why I do it.  There is negligible difference in performance good or bad for those 100rpms, thus I prefer them out because it is a lot quieter.
