Yes, but there are procedures for everything in the military.  Everything.  To include avoidance of collisions at sea, I'll bet.

I'd guess the senior watch officer on duty is the one in trouble

Responsible yes,but not necessarily held accountable for the actions of officers under him.Read up on the Calley case
